Training Element Record (Pre-printed)

This form is used to record the training of each operator and ensure that all elements of training are recorded and agreed by both the instructor and the trainee. It records all details of the course including dates, truck type, make, model, capacity, max height etc. On the reverse of the sheet is an individual training feedback form where trainees can evaluate the course and give feedback for possible improvements.
